Can I get English documents?
The medical questionnaire (문진표)
This is the first form you fill out. It asks about medical history, allergies, current medications, and your main complaint today.
Consent forms (동의서)
Before procedures or tests, you'll sign a consent form. Make sure you understand what you're consenting to — ask for explanation if needed.
Medical certificates (진단서)
You may need a 진단서 for insurance claims, work absence, or legal purposes. Ask at the reception desk — there's usually a fee of 3,000-10,000 won.
Prescriptions (처방전)
Take your prescription to any pharmacy. It's valid for 3 days from the date of issue.
Tip
If you need documents in English (for insurance claims back home, visa purposes, etc.), ask: 영문 서류 가능한가요? Most large hospitals can provide English versions.
